Resident Evil Requiem has five dial safes that need a code, plus one already-open safe and a three-letter briefcase. On Casual and Standard the codes are fixed for every player: the Bar & Lounge safe is Left 10, Right 80, Left 30 and the Examination Room safe is Right 30, Left 10, Right 50. Insanity uses a different set of combinations, and the dial directions flip as well. Both sets are in the table below.
All five dial safes belong to Grace’s sections: three in the Rhodes Hill Care Center and two in the ARK. The briefcase is in the RPD and opens during Leon’s part of the game. Every code below was cross-checked against PowerPyx, PC Gamer, Push Square, VGC and PCGamesN (checked October 30, 2026 UTC). Where those guides disagree, we say so. Locations are kept spoiler-light.

*Two Insanity entries are disputed. For the Bar & Lounge safe, PowerPyx, VGC and Beebom list Right 20, Left 50, Right 30, but PCGamesN says the normal code still works on Insanity. Try Right 20, Left 50, Right 30 first, then the normal code. For the Sterilization Chamber, four guides end on Left 0 and only Beebom ends on Left 20.
Guides also report different ammo counts for the Basement and Sterilization Chamber safes (x10, x15 or 30), so the table names the items without quantities.
How to enter a safe code in Resident Evil Requiem
Each combination is three numbers, and the direction matters as much as the number. According to VGC, Left means clockwise and Right means counter-clockwise, and FinalBoss adds that an on-screen indicator shows which way you are turning. Turn to the first number, reverse to the second, reverse again to the third, and only then press Check. PowerPyx notes that a wrong number or direction just spins the dial back to its default position, so you can retry as often as you like.
You do not need to find the code files first. PowerPyx and PCGamesN both say the codes are always the same on Casual and Standard and are not randomised, so you can enter them as soon as you reach a safe. The files are still worth picking up if you are going for the document achievements.

Rhodes Hill Care Center safe codes
Bar & Lounge safe code: Left 10, Right 80, Left 30
This is the first safe most players see. It sits on the counter behind the bar, in the room with the singing zombie in a white dress, which the story takes you through anyway. Inside are 3 Antique Coins for the Parlor upgrades. PC Gamer also points out Grace’s S&S M232 pistol behind the same bar and a spare Antique Coin on the piano stool. Its code file, “Bar & Lounge Safe Code”, turns up later, but you can open the safe without it.
Examination Room safe code: Right 30, Left 10, Right 50
Once a story keycard opens the East Wing, the Examination Room safe is inside a cabinet in the corner of the room. It holds another 3 Antique Coins. PC Gamer says the paper with the combination is near the Lead Researcher’s Office.
Waiting Room safe: no code needed
Behind the admin desk in the Waiting Room, just past the East Wing Lobby shortcut, is a safe that is already open. VGC lists 2 Antique Coins inside, and PC Gamer says “a couple”. VGC says it is empty on Insanity.
Basement safe code: Right 60, Left 40, Right 80
This is the most missable safe, and the one to prioritise. Take the Joint Plug from the Holding Cells Corridor and use it on the Furnace room door. The safe is in a side office on the right of the next room, where the forklift is. You only get this far after the game has switched to Leon and back to Grace.
Its code file is in the attic during a Leon section. VGC and PowerPyx say you should already have it by now, while FinalBoss says it only turns up later. Either way, the code above works without the file. The reward is the Raccoon Roundup Map (Rhodes Hill), which marks every Mr. Raccoon statue in the Care Center, plus handgun ammo. Grab any statues you missed before Grace leaves the area.
ARK safe codes (late game)
Both remaining safes come much later, in the ARK, again as Grace. The directions below only name rooms.
Sterilization Chamber safe code: Right 10, Left 90, Right 20
The safe sits by the window directly in front of you as you enter the Sterilization Chamber. PowerPyx says this room has the first Licker you meet, and suggests sneaking around it rather than fighting. According to VGC and FinalBoss, the code file is in a container near Bio-Weapon Storage 05. Inside: a Med Injector, a Molotov Cocktail and handgun ammo.
Monitor Control Room safe code: Left 50, Right 60, Left 80
The Monitor Control Room is just off the Lobby with the red floor paint. According to PowerPyx, you use the Magnetic Key story item on the red door by the upstairs typewriter, enter the first green door on the left, and find the safe by the monitors. The code file is in one of the containers in Bio-Weapon Storage 05. It holds a Med Injector, a Bottle of Acid and an Empty Bottle.

The briefcase in the S.T.A.R.S. Office uses three letters instead of a dial. The code is RRR, and PC Gamer, Destructoid and Shacknews all agree on it. The case holds the Power Shades Charm, which goes on Leon’s Requiem revolver.
Here is the clue chain, per Destructoid and PC Gamer:
- Read Barry’s To-Do List on Barry’s desk in the S.T.A.R.S. Office. It mentions a missing library book that “the captain” checked out last.
- Check the drawer of Wesker’s desk in his office. A Library Card shows he borrowed Medicinal Benefits of Herbs.
- Find that book on the upper floor of the RPD Library. Inside is a photo of Rebecca Chambers, the classic Resident Evil 2 easter egg.
- Flip the photo: “Rising Rookie Rebecca” gives R, R, R.
Does the briefcase count toward No Safe is Safe? PowerPyx counts five safes for the achievement, while FinalBoss says the briefcase is needed as well. It sits on Leon’s route anyway, so open it to be safe.

The achievement is single-run in practice. Because everything in Requiem is missable without chapter select, the Basement safe is the one that most often breaks a cleanup run.
