Animal Hospital anomalies come in three groups: ones you can see at the reception window, ones that only show up in the patient’s photo, and ones that only appear on the security cameras. The community wiki documents 19 anomaly signs across those groups, plus 10 enemies that attack inside the hospital. The game also has 13 classes, from the free Intern to the Robux-only Secret Agent. Facts were checked on October 28, 2026 (13:45 UTC) against the game’s Roblox page, its official update listings and the Animal Hospital Wiki.

Quick answer
- The game: “Animal Hospital (Anomaly)” by the Roblox group Animal Anomaly. It launched on May 10, 2026 and has 2.48 billion visits, with 57,344 people playing when we checked.
- Check every patient three ways: look at them through the window, take a photo and let it develop, then watch them on CCTV. If anything doesn’t match, keep the shutters closed.
- Classes: 13 in total. Eleven cost Animal Coins (Nurse 20 up to Surgeon 2,500, with Intern free). Head Nurse and Secret Agent cost Robux.
- Common advice from guides: buy Secretary (120 coins) first, save up for Surgeon, and pick Secret Agent if you spend Robux.
- Halloween: the official “Halloween!” event runs until November 14, 17:00 UTC, and “Halloween – Part 2” starts October 31 at 19:00 UTC.

The official game description sums up the premise: you work the night shift treating patients, “but some patients are acting weird…”. The game has one badge, “Animal Hospital x The Hollow”, which Roblox’s badges API shows was awarded about 1.2 million times. That’s the crossover with Roblox’s Halloween event, covered in our Roblox The Hollow event guide.
How to spot anomalies in Animal Hospital
Every guide we checked, and the wiki, gives the same routine:
- Keep the shutters closed while the patient stands at the reception window.
- Look at the patient: check the face, eyes, teeth and ears, and watch for a few seconds to catch twitching.
- Take a photo and wait for it to develop. According to the wiki, picking up a cursed photo before it finishes developing costs 10 sanity.
- Compare the photo with the patient: eyes, face, ear shape and ear color.
- Check the security cameras. The lobby camera gives a side view, which the wiki recommends for spotting a Security Camera Mimic.
- Open the shutters only if all three checks are clean. If not, leave them closed and the patient walks away.
The wiki also says flickering lights can sometimes mean an anomaly is at the window, even when you see nothing else.
Every Animal Hospital anomaly (full list)
The names below are the wiki’s. Guides often use different names for the same anomaly, so we list those too. The Spike counts “20 documented patient anomalies” because it splits the photo mismatches into separate entries. Beebom’s list was last updated in July, before the Tiny Anomaly existed. The developer hasn’t published an official list. The in-game Journal, added on July 25, records the anomalies you’ve encountered.
Appearance anomalies (visible at the window)
| Anomaly | Also called | How to spot it |
|---|---|---|
| Smiling with Creepy Eyes | Wide Eyes and Creepy Smile, Unnatural Grin | Big eyes with black pupils and under-eye bags, plus a huge smile that looks drawn on |
| Three Eyes | – | Three glowing red eyes, slightly off center, in place of the normal eyes |
| Crooked Face | Mismatched Eyes and Sharp Teeth | A crooked smile full of sharp teeth, with mismatched eyes sitting high on the face |
| X-rayed Face | Human Teeth | Small, wide circular eyes and realistic teeth |
| Staring | Wandering Eyes | The head follows you, turning up to about 60 degrees either way |
| Hollow Face | – | Dark hollow eyes, a hunched posture and twitching |
| Twitching | – | Twitching arms, head and neck, with a hunched back |
| Tiny Anomaly | – | A very small pink bunny with three red eyes and a high-pitched voice (added July 25) |
Two of these behave unusually. According to the wiki, Hollow Face anomalies don’t attack. Instead they always start a Death Ritual while they recover. You put out the candles with IV Drops, Eye Drops or coffee to avoid losing sanity. Staring is also tricky: Ratthew and the Shopkeeper Nurse stare at you the same way, but they aren’t anomalies.
Photo anomalies (visible in the developed photo)
| Anomaly | How to spot it |
|---|---|
| Incorrect Photo | The photo doesn’t match the patient: different face, eyes, ear or horn shape, or ear color. Guides list Different Eyes and Different Ears as separate entries |
| Static Photo | The photo has a grainy static effect. The wiki warns that after shift 10 it isn’t a guaranteed anomaly, so close the shutters rather than tasing or shooting |
| Cursed Photo | A disturbing face shows up only in the photo (the wiki has six variants). Don’t pick it up while it’s still developing |
Security camera anomalies (visible only on CCTV)
| Anomaly | Also called | How to spot it |
|---|---|---|
| Black Eyes | Censored Eyes, Black Bar Eyes | A black bar covers the head, with realistic eyes staring out |
| Unnatural Body | Contortionist, “boneless” | A stretched, distorted body that changes shape when you reopen the camera |
| Staring at Cameras | – | Stares into the CCTV camera at all times, even while walking |
| Void/Black | Void Body | The whole body shows up pure black on camera |
| Camera Twitching | – | Twitches on the feed only. Watch for a few seconds |
| Security Camera Mimic | Skinwalker, Disguised Shapeshifter | Shows as a Mimic on camera once it reaches the check-in spot |
| Mismatching Ears | Camera Mismatched Ears | The ears or horns on camera differ from the ones you see at the window |
| Mismatching Face | – | A different face on camera, only on your first look. The wiki says this may be a bug |
What happens if you let an anomaly in?
According to the wiki, an admitted anomaly usually says “I’m hungry…” after treatment and may attack a patient or a player. It can also start a Death Ritual, which gives you 40 seconds to remove every candle. In room 8 it can trigger Monster Emerging, where tentacles burst out and you have 45 seconds to finish the surgery: scissors, transplant, scissors, transplant, then three random treatments. Don’t tase or shoot the tentacles, because that kills the patient. Other useful details from the wiki:
- Tasing an anomaly gives you +2 sanity. Tasing a normal patient costs -15.
- Anomalies never turn hostile on the second shift.
- If every patient is treated but “The doctor is arriving” never shows up, an anomaly is still inside.

All Animal Hospital enemies and how to counter them
Enemies are separate from patient anomalies. They appear inside the hospital, and closing the shutters doesn’t stop them. Sanity damage and counters below come from the wiki’s enemy pages. Beebom and The Spike recommend the same counters.
| Enemy | Where | Sanity damage | Counter |
|---|---|---|---|
| Mimic | Rooms 1-8, reception | -15 on attack, then about -6 per bite | Gun, taser, or mash E to break free |
| Head Banger | Reception side window (shifts 2-5) | -20 if you send it away without coffee | Give it coffee, or wait about a minute until the glass breaks |
| Bed Monster | Rooms 1-5 and 7 | -10 when it grabs you, then about -5 to -6 every 2 seconds | Maple Syrup, Organ or Transplant. Stay out of its red zone |
| Stalker | Hallway corners | -10 when you look at it | Don’t make eye contact. A camera flash also works |
| Hiders | Patient rooms, both wings, Lab | -10 if you’re empty-handed (otherwise it steals your item) | Fire Extinguisher, taser or gun |
| Mass of Eyes | Ceiling (“Don’t Look Up”) | -3 to -5 per second while you look at it | Don’t look up. Eye drops also work |
| Camera Monster | Broken security cameras | -20 on jumpscare | Switch feeds fast or fix the camera |
| Ghost | Anywhere, only after a patient dies | -2 on contact, -2 per second nearby | Taser, gun or scanner. A Fire Extinguisher reveals it |
| Tentacle | Room 8 surgery | Drains sanity if you stand inside it | Finish the surgery order within 45 seconds |
| Slimebert | Slime Shift (around shift 10+) | -20 when it slimes you, -3 while digested | Fire Extinguisher stuns it. Cough Syrup also works |
All 13 Animal Hospital classes
You buy classes in the lobby’s Classes shop with Animal Coins (earned from shifts: 2 per patient treated, according to the wiki) or with Robux. Every class has three levels. Level 2 costs 200 EXP or 199 Robux, and level 3 costs another 850 EXP or 299 Robux. Starting sanity without a class bonus is about 60-65%. The official September 19 update listing confirmed “new skins and classes”, which were the Scientist and Barista. Their prices, and the Firefighter’s, come from the wiki only, because the guides we checked predate them.
| Class | Cost | Lv. 1 | Lv. 2 | Lv. 3 |
|---|---|---|---|---|
| Intern | Free (starter) | +10 starting sanity | +15 sanity | +20 sanity |
| Nurse | 20 coins | +1 inventory slot | +2 slots | +3 slots |
| Secretary | 120 coins ($20 start) | +1 sanity per check-in | +5 starting sanity | +10 starting sanity |
| Paramedic | 250 coins | Large Run Fast Cola (6 uses) | 9 uses | Regains 1 use per round |
| Psychologist | 500 coins | Starts at 100% sanity; all sanity effects doubled | 7.5% chance to ignore sanity loss | See teammates’ sanity |

| Firefighter | 1,100 coins | Fire Extinguisher, +1 inventory slot | +1 sanity per fire put out | Immune to fire drain, +10 sanity |
| Scientist | 1,250 coins | Super DNA Sampler (8 slots), 2 samples per extraction | Lab cultures grow 3x faster | +1 sanity per potion press |
| Security | 1,250 coins ($20 start) | X-Taser (5 uses) | Adds a Deployable Camera | Taser regains 1 use per round |
| Barista | 2,000 coins ($30 start) | Starts with coffee; brews special coffees | Give coffee to teammates | Machine cools down 15% faster |
| Surgeon | 2,500 coins ($35 start) | Sanity + 4-second speed boost per heal | 8-second boost | 2x sanity per heal, +10 starting sanity |
| Head Nurse | About 190 Robux (regional) | +3 inventory slots ($30 start) | +10 starting sanity | Starts with every treatment (“Special Technique”) |
| Secret Agent | About 890 Robux (360-1,070 regional) | Gun (20 uses), multi-use scanner, +2 carry slots | Gun has 24 uses, regains 1 per round | Gun kills on anomalies give +4 sanity and +5 cash |
Add up the wiki prices and every coin class costs 9,890 Animal Coins in total, plus the two Robux classes. The Barista brews special coffees from ingredients; the wiki lists Chocolate (Cappuccino, 21 sanity per use) as the strongest.

Best class in Animal Hospital
There’s no official ranking, but the guides mostly agree. Pocket Gamer tells free-to-play players to “save 120 Coins and buy the Secretary” first. TechNerdiness calls Surgeon “the go-to for free-to-play players” and says Secret Agent handles anomalies “more reliably than any other class”. Both guides were written in July, before Firefighter, Scientist and Barista were added. Going by the perks in the table, a sensible order looks like this:
- Secretary (120): cheap, and you regain sanity on every check-in.
- Doctor (900) or Surgeon (2,500): sanity on every heal. Surgeon also gives speed boosts.
- Barista (2,000) for team runs. From level 2 you can hand coffee to teammates.
- Secret Agent if you spend Robux. Its gun and scanner make anomaly control much easier.
Animal Hospital Halloween event
The official “Halloween!” listing runs from October 17, 18:00 UTC until November 14, 17:00 UTC, and a separate “Halloween – Part 2” listing starts October 31 at 19:00 UTC. Here’s what the wiki’s Halloween Event page says:
- Trick-or-Treaters ring a bell at a stand next to the office and ask for up to three candies. Each candy you give earns 1 Halloween Coin.
- Some of them are anomalies in costume. Take a photo with the tombstone camera and check it through the cauldron. If you confirm an anomaly, press “TRICK” to send it away for 1 coin. Giving it an Organ or Transplant from room 8 earns 3 coins and the “Meat!” achievement.
- Halloween skins cost from 125 Halloween Coins (Spooky Intern) to 850 (Agent Count Dracula).
